The open enrollment period runs from June 1 through June 30. Health plan change forms are accepted during this
period. No major changes to your health selections have been made this year. When you are ready to submit this open enrollment form (SCROLL DOWN TO VIEW), be sure you have checked and
updated your dependent data through the Members Login function. Some of our health carriers will only accept additions (other than newborns) during open enrollment. Health care costs
continue to rise. In order to maintain the long term success of your health and welfare plan, your Trustees continue to monitor and adjust the level and type of benefits which are made available to
